It  truly is a sad time when a loved one passes. Creating a memorial plaque is a thoughtful way to remember your loved one that can be placed in your home on the wall or on an easel. In addition to their name, date of birth and passing date, we can  insert your favorite scripture or sympathy quote you like on the bottom of the plaque.

By default, the memorial service information and location is NOT placed on the plaque. If you want it inserted on the plaque, you must check the instruction boxes below advising us to do so. Your memorial is reproduced as an image minus any embossed elements or boarders. If you have any other instructions for us, please type the request in the special instructions box. We will do our best to accommodate them.

We try to match the colors as close as possible, but it will not be an exact match when it is applied to the plaque.

Once the order is processed, there are no cancellations, so please order carefully.

If you have a digital image of the announcement, please use the below upload box to send us the image.

If you don't have a digital image and you can scan the item, it can be scanned and saved as a .jpg and in 300 DPI resolution. This can be uploaded or emailed to: images@UniqueDeocrativeWallDecor.com with a note of your Sales Order number.

To insure we receive the document, it is preferred to send it with a UPS tracking number. We are not liable if the document is damaged or lost in the mail.

The normal delivery time for this custom plaque is 2~3 weeks from the time we receive the image or physical invitation/announcement.
